Enrolment process

Reach out

Reach out to Head of Element, Andrea Deans. The Element enquiry made by school or Local Authority. Support required and potential placement discussed. Referral form completed. Single point of contact confirmed.

Connect with your CYP

Face to face induction meeting with CYP, their parent/carer and the school/referrer prior to accepting the referral

03.

Site Visit

CYP and parent / carer offered an opportunity to visit the site so that they can become familiar with the environment and meet key staff.

04.

Programme Overview

Programme overview and timetable sent to school by the Head of Element.

05.

Agree Targets

Agree targets and support plan for the placement with school.

06.

Service Level Agreement

Service Level Agreement signed and shared with all parties.

07.

Reviews

Mid-point review (KS2 / KS3 – 6 school weeks from start date), visits from school, success criteria and reintegration plan (KS2 / KS3) agreed.

08.

Weekly Updates

Weekly report shared with school and home.
